What Is Auvelity?
Auvelity is an antidepressant medicine used to treat Major Depressive Disorder (MDD) which is a specific type of depression.
It is a rapid-acting antidepressant that started working as early as 1 week, and patients’ depression symptoms continued to improve over time when compared to placebo in a 6-week clinical study for MDD.
It contains two active ingredients, Bupropion and Dextromethorphan. It is thought to work by increasing the amount of glutamate (a chemical messenger) available in your brain, which may positively affect your mood and ease depressive symptoms.
It may also weakly affect your brain’s other neurotransmitters, norepinephrine and dopamine.
Dextromethorphan and Bupropion has been approved by the US Food and Drug Administration (FDA) on 18th August 2022 for treating MDD in adults.
It is the first and only oral medication with a fast onset of action that has been approved for the treatment of MDD and carries a label indicating statistically significant antidepressant efficacy starting at one week when compared to a placebo.
How Does Auvelity Work for MDD?
Auvelity drug helps to lessen the symptoms of major depressive disorder through a combination of two medicines that have antidepressant effects.
Bupropion is an antidepressant that causes an increase in the chemicals dopamine and norepinephrine in the brain.
Dextromethorphan, a cough suppressant that is found in many over-the-counter cough medicines, also changes mood-related chemicals in the brain. The combination of these two medicines treats depression symptoms better than bupropion alone.
Auvelity Safety and Tolerability
AUVELITY was evaluated for safety in over 1100 patients with MDD at the time of approval.
A total of 1114 patients were evaluated from four clinical studies (two 6-week studies in MDD, one 6-week study in another indication, and one long-term study in MDD and another indication).
Overall, the GEMINI trial showed 62% of patients receiving AUVELITY experienced any adverse event compared to 45% receiving placebo.
The most common adverse reactions (≥ 5% and more than twice as frequently as placebo) include dizziness, headache, diarrhea, somnolence, dry mouth, sexual dysfunction, and hyperhidrosis.
Four percent of AUVELITY-treated patients discontinued study participation due to an adverse event compared to 0% of placebo-treated patients.
At Week 6, Dextromethorphan and Bupropion was associated with a 0.44 lb weight loss compared to a 0.88 lb weight gain in placebo-treated patients.
Contraindications to taking AUVELITY include:
- Seizure disorder
- Current or prior diagnosis of bulimia or anorexia nervosa
- Undergoing abrupt discontinuation of alcohol, benzodiazepines, barbiturates, and antiepileptic drugs
- Use with an MAOI or within 14 days of stopping treatment with AUVELITY. Do not use AUVELITY within 14 days of discontinuing an MAOI
- Known hypersensitivity to bupropion, dextromethorphan, or any component of AUVELITY
Auvelity Dosing and Administration
AUVELITY is an oral antidepressant treatment available in one dosage strength with a recommended therapeutic dose by day 4.
Prior to starting therapy and during treatment with it, it is important to assess blood pressure and monitor periodically during treatment; to screen patients for a personal or family history of bipolar disorder, mania, or hypomania; and to screen for current use of bupropion or dextromethorphan in other medications.
The recommended starting dose of it is 1 tablet (45 mg/105 mg dextromethorphan/bupropion) once daily in the morning.
After 3 days, increase to 1 tablet twice daily, separated by at least 8 hours, which is the maximum recommended daily dose.
Tablets should be swallowed whole and not crushed, divided, or chewed. AUVELITY can be taken with or without food.
The recommended dosage of Dextromethorphan and Bupropion is reduced to 1 tablet by mouth daily in the morning:
- In poor CYP2D6 metabolizers
- In moderate renal impairment
(eGFR 30 to 59 mL/minute/1.73 m2). - When co-administered with strong CYP2D6 inhibitors
Common Auvelity Side Effects
The most common Dextromethorphan and Bupropion side effects include:
- dizziness
- diarrhea
- dry mouth
- excessive sweating
- headache
- feeling sleepy
- sexual function problems
Auvelity medicine may cause serious side effects, including: seizures, hypertension, manic episodes, unusual thoughts or behaviors, eye problems (angle-closure glaucoma), dizziness, serotonin syndrome.
Important Warnings
Before you start treatment, tell your healthcare provider about all your medical conditions, including if you:
- have or had seizures or convulsions
- have had a head injury
- have had a heart attack or have heart problems
- have a tumor in your nervous system (brain or spine)
- have had a stroke
- have low blood sugar
- have low sodium levels in your blood
- have liver problems
- have kidney problems
- drink a lot of alcohol
- abuse prescription medicines or street drugs
- have diabetes and take oral diabetes medicines or use insulin to control your blood sugar
- have high blood pressure
- have history of falls
- have or had bipolar disorder, or have a family history of bipolar disorder, suicide, or depression
- have high pressure in the eye (glaucoma)
Pregnancy & Breastfeeding
Tell your healthcare provider if you are pregnant or plan to become pregnant. Auvelity may harm your unborn baby if you take it during pregnancy.
It is not recommended during pregnancy. And do not breastfeed during treatment with this medicine and for 5 days after the final dose.
